SPORTS PHYSICALS

Sports physicals can be administered by a doctor, physician’s assistant, licensed nurse, or nurse practitioner. The sports physical form provided by TSSAA must be used, and can be found below or at TSSAA.org. Physical forms should not be dated before April 21, 2012, and can be used for all sports through the 2012-20123 school year. The Little Clinic inside Kroger offers physicals for $35
